Assumption of Risks 

I, the undersigned, understand and acknowledge that participation in ninja warrior, parkour, aerobics, fitness and/or associated activities (collectively the “Activities”) might result in  personal injury, death, property damage, expense and related loss, including loss of income. I fully understand  these risks and hereby agree to participation voluntarily and at my own risk. I further state that I am in proper  physical condition to participate in the Activities.

Liability Waiver and Release: 

In consideration of the acceptance of my attendance and/or participation at Grande Prairie Ninja Warrior, I waive any and all  claims I may have against, and release from all liability, and agree not to sue Grande Prairie Ninja Warrior, its directors, officers,  officials, servants, agents, sponsors, employees, and volunteers for any personal injury, death, property damage  or loss sustained as a result of my attendance and/or participation at Grande Prairie Ninja Warrior, arising out of any cause  whatsoever including without limitation negligence or breach of statutory duty. 

I agree to indemnify and hold harmless Grande Prairie Ninja Warrior, its directors, officers, officials, servants, agents, sponsors,  employees, and volunteers from any and all third party claims, demands, actions or costs (including legal costs  on a solicitor client basis) for which I am legally responsible arising out of or in consequence of my attendance  and participation in the Activities.  

Media Release: 

I give my permission for the free use of my likeness in photographs and video. These images may appear in  publications, promotional material, internal staff training manuals, and on the internet in association with Grande Prairie Ninja Warrior. 

I understand and agree that these images will become the property of Grande Prairie Ninja Warrior and I hereby irrevocably  authorize Grande Prairie Ninja Warrior to edit, copy, exhibit, publish or distribute these images for any lawful and moral  purpose. 

Membership Agreement

I understand that if I do not give Grande Prairie Ninja Warrior 2 weeks notice for cancellation of membership that I will be charged for the upcoming month. The notice must be made by either email, facebook page message, or messaging the website so that both parties have the cancellation in writing.

I understand and acknowledge that there is a $20 late fee applied to membership pricing for any membership that isn't paid before the 6th of each month that doesn't have prior approval from Ownership/Management of Grande Prairie Ninja Warrior.

ACKNOWLEDGEMENT: 

I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD THIS AGREEMENT, that I have executed  this agreement voluntarily, and that this agreement is to be binding upon me, my heirs, executors, administrators  and representatives.
